Youth Care Coordination
Purpose: To assist Region II families who have a child with severe emotional disturbances and significant functional impairment find and access needed services to maintain the child in the home and community.
- Level A- MH
Youth Care Coordination, Level A uses the wraparound philosophy to develop a team of individuals to wraparound the youth and family to maintain the child in the home. The team recognizes and uses the family’s strengths to develop a plan of care specific to their needs. Region II will provide services to dual diagnosis youth. - Level B- MH
Youth Care Coordination, Level B also uses the wraparound philosophy to develop a team to support families with a child with a mental health diagnosis. This is for families who would benefit from a lower level of care and help with the coordination of services. Region II will provide services to dual diagnosis youth. - Level T- Intensive Case Management- MH and SA
Youth Care Coordination, Level T is a program for adolescent and young adults with a severe mental health and/or substance abuse diagnosis who are moving into the adult system. We provide wraparound support helping them experience a seamless transition to the adult behavioral health system.
